Bad Root Canals Replaced with Implants

Infected upper molar root canals #14, 15 replaced in stages with two dental implants:

  1. Extract bad root canal teeth and do socket bone graft
  2. Wait 4 months. X-ray (CBCT) to make sure the bone has healed.
  3. Place both dental implants in ideal position.
  4. Wait 3-4 months for implant bone integration
  5. Restore both implants with custom abutments and screw-retained zirconia crowns

Upper molars need time! Sometimes I place the implants at the same time of extraction with an internal sinus lift as long as the bone it is safe to do so. Each case is unique! Root canal treatment sometimes slows the process of having an implant. Retreating the root canals does not always work.

Note the ideal shape and contours of the dental implant teeth. This is important to to long-term outcome since implants are NOT permanent! Root canals are not permanent either ;-(

    Do you believe a cantilever tooth attached to Implant #8 would work? Would that be too much load on implant #8? I don’t have many options here. I suppose a denture for number 7 would be a last option. Thank you for your time. I appreciate your opinion.

    • Better to use #6 as an anchor than #8. A cantilever bridge from the canine to the lateral in dental terms refers to a dental bridge that is anchored on the canine tooth and extends to replace the missing lateral incisor without support on the other side of the gap. The canine serves as the single abutment tooth, and the bridge spans from this tooth to the space of the missing lateral, effectively “cantilevering” the replacement tooth (pontic) into place.

  2. Nice! Is the stuff next to the root of #15 implant extruded sealer or GP from the failed root canal? Is it OK if there are still remnants there or does it all need to be removed before the implant is placed?

  3. Dear Dr. Amin,
    Today, I saw a prosthodontist that I’ve seen in the past. Tooth # 7 has hurt for 2 years, after tooth #8 was removed and an implant/crown placed. We discussed removal of #7 and putting a cantilever tooth on the implant at # 8. With the right occlusion, he said it was a reasonable option. Option #2 is to take a scan and see if there’s enough room for a mini (3 mm) implant to replace #7. Are either of these options viable for the long term? I’ve read many negative reviews regarding mini implants. Of course, I don’t know if there’s enough room until I get the scans, but assuming there is enough room, is a mini implant an good option? A third option is to try splinting my upper jaw for 2-4 months to see if that would improve pain on #7. That is an expensive option if it does not work ($6200). Thank you for your advice Dr. Amin.
